Student to Parent Questionnaire
Within your Google Drive you will need to select New, then Google Form.
The link above is useful if you have not yet used Google Forms. Teach yourself how to use the forms before tasking the students with building their own parent questionnaire.
Here is a form that I have started as an example. You will need to decide how long the questionnaire will be depending on your classroom and students. When the form has been compiled, students can email it to their parents, open it up at home with their parents, or print it off at school and hand-write it.
